New Patients Registration Patients requesting to join the practice will be asked to do the following: Confirm their identity and residency in the UK (birth certificate or passport and utilities bill or bank statement) Complete their address on the medical card (if they have one) Complete a GMS1 registration form Complete a short Health Questionnaire which will then be screened by the Practice Nurses/ Health Care Assistant. Any information contained in the questionnaire which would indicate the need for an appointment with either a GP or the Practice Nurse will be followed up with a letter or telephone call to the patient to book an appointment. Patients can ask for a New Patient Health check should they wish to and this will include measuring height, weight and blood pressure, and routine urine testing.
